Coverage Detemination
We do accept some insurance plans which we have contracted. Some of these plans do require a pre-authorization/referral from your primary care provider for you to recieve maximum benefits.

If you do not see your insurance plan listed, then please call the office to check for current status on your coverage eligibility. If we are not on your insurance plan we will submit to your insurance plan out-of-network for your benefits and we do offer discounts for our patients with no insurance coverage.You may call the office for further details
